Scheduling:

I normally see patients by appointment only. Please call my clinic at (818) 900-2301 to arrange for an appointment.  We generally do not accept walk-insCall if we have same-day appointment slots available.

Late or missed appointments:
Occasionally, unforeseen circumstances can arise that may prevent you from keeping your appointment. If you are late less than 15 minutes for an appointment, I will first see other patients who have shown up for their set time, and then see you at the first available opportunity. 

 

If you are late more than 15 minutes for an appointment, we will reschedule your appointment to a different day. 

* As a courtesy, please call us if you need to cancel or reschedule your appointment. *

If you miss an appointment without prior notification a 3rd time or more, or have a habitual pattern of missing appointments without prior notification, I may request you to transfer care to a different clinic. 

 

Our policies ensure that we keep our appointments timely, handle urgent requests and accommodate other patients on our waiting list.

Prescription refills:
If you need a medication refill that I prescribed for a chronic condition, please call the pharmacy first, who will get in touch with me. If I have not seen you within the past year, you will need to schedule an appointment to renew the prescription. If you need a refill for a prescription that you do not take regularly or was prescribed by a different doctor, you will also need to schedule a new appointment.


Except for a few situations, I will not prescribe new medications over the telephone. For medico-legal reasons, I need to physically evaluate you first whenever I write a new prescription.

Insurance plans:
It is the patient's responsibility to know the details of his/her insurance coverage. Please check with your employer, insurance agent, or other appropriate person. It is your responsibility to notify me or my staff for any changes in your medical coverage.

Payments for co-insurances and other fees:

I only accept payment by credit/debit cards or cash. We don't accept checks.

Billing for preventative visits:

Annual Physical checkups usually have no co-pays, depending on your insurance. However, during your routine physical exam, if you request additional consultation for a medical problem, you will be charged a co-pay. For example, if you come in for your annual physical and you request an evaluation for a respiratory infection, you will be charged a co-pay.
