PROCEDURE:
1. Download the REGISTRATION FORM and HEALTH QUESTIONNAIRE.
2. Print out forms and fill up manually. Sign and date where indicated.
3. Submit forms through our secure portal (call clinic for instructions); or fax to (818) 900 - 2471.
